If you or your loved one are preparing to leave a healthcare facility and return
home, we are available to meet with facility staff and attend Discharge Planning
meetings to promote a smooth transition and sound continuity of care. After
discharge, our Caregivers will follow prescribed exercise programs, provide
medication reminders, monitor vital signs, meal intake and bowel and bladder
habits.
Our comprehensive Case Records, which are maintained in our Clients' homes,
provide detailed information regarding each Caregiver's visit, allowing family
members the opportunity to monitor their loved one's daily activities and
progress. We include a large-format Emergency Contact Sheet for your
convenience - a simple list of telephone numbers including Physician, Pharmacy
and family which may be removed from the Case Record and posted next to a
telephone for easy access. The front page of every Case Record is a detailed
Face Sheet containing all pertinent medical and contact information for our
client in the event of a medical emergency.
We have also created a detailed Home Safety Profile, designed to identify
danger areas in your home and simple solutions that you can put in place to
minimize the risk of fall or accident.
